What to do if you are exposed

You are considered exposed to a person positive for COVID-19 if you were less than 6 feet away from someone with COVID-19 for a total of 15 minutes or more over a 24-hour period, two days before their symptoms start or before their positive test.

Effective 2/10/22, Allen County Public Health will provide self-attestation quarantine letters to Allen County residents who need them through this website. You do not need to contact the health department to use this self-attest letter.

Please use the link below for a fillable quarantine letter to be used as needed for return to work or school.

Anyone that is identified as a close contact should get tested because of the potential for asymptomatic and pre-symptomatic transmission.

PLEASE DO NOT WAIT TO HEAR FROM THE HEALTH DEPARTMENT TO BEGIN QUARANTINE OR ISOLATION!
