Allen County Public Health can provide birth and death certificates from December 1908 to present as long as the following conditions are met:
- Individual was born in the state of Ohio from December 1908 to present
- Individual died in Allen County from December 1908 to present, or died in Ohio from 1971 to present
- For birth certificates you must have: full name at birth, date of birth, and parents’ names (including mother’s maiden name)
- For death certificates you must have: full name at time of death & date of death
Certified Copies are $27.00 per copy
We accept:
- Cash, Money Order, Cashier’s Check
- Debit/Credit (minimum $2.50 usage fee applies)
- Business Checks accepted **No Personal Checks are accepted**

- Include $27.00 fee for each certificate (No personal checks)
