Function/Purpose:
Our Reproductive Health and Wellness program, known as the Family Planning Clinic,
provides quality confidential gynecological and contraceptive services to
men and women regardless of financial situations. Education and counseling
are provided regarding all contraceptive choices and family planning issues.
Services
Family Planning provides access to:
- Annual screening exams
- Pap testing
- Breast exams
- Screening tests for Sexually Transmitted Infections and treatments for infections as needed
- HIV testing
- Pregnancy testing and counseling with referral for care as needed
- Contraceptive products
- Referral for other healthcare services.
Family Planning offers pregnancy testing by appointment.
Care is provided by an expert staff of licensed medical professionals including Physicians, Nurse Practitioners, Registered Nurses and a very caring support staff who are available to help answer all your healthcare questions.
Family planning presentations can be given by Allen County Public Health nurses upon request.
Cost
Fees apply and are assessed based on HOUSEHOLD INCOME information using the federal poverty guidelines. Medicaid accepted. Medicaid applications available. We are a participating provider for Anthem Blue Cross/Blue Shield, Aetna, United Healthcare, and Medical Mutual, AmBetter, Cigna, and Paramount PPO.
Payment methods – Cash, check, and credit card. There is a $2.50 minimum fee for credit cards. No one denied service due to inability to pay. Phone payment options are also available.
